This signed Authority and Mandate relates to our agreement as of the date of signature (“the Agreement”).I/We hereby authorise My Rewards Club to issue and deliver payment instructions to my/our bank for collection against the account listed below.
I/We authorise my/our bank to process these payment instructions against my/our account (or any other bank account I/we may transfer to), provided that such payment instructions do not exceed my/our obligations under the Agreement.
This mandate will commence on the agreed commencement date and will continue until terminated by me/us with 30 (thirty) ordinary working days’ written notice, delivered either via prepaid registered post or to the physical address listed above.
